Photobiomodulation for TBI therapy uses near-infrared (NIR) light, typically in the 600- to 1100-nm wavelength range, and is believed to elicit biostimulation mediated by mitochondrial light absorption. Specifically, cytochrome C oxidase, a large trans-membrane protein complex that is a part of the respiratory electron transport chain, is thought to absorb the light and upregulate adenosine triphosphate production.The NIR may also upregulate messenger molecules, including reactive oxygen species and nitric oxide, which in turn activate other transcription factors, such as nuclear factor-κβ and activator protein-1, that enter the nucleus and cause transcription of a range of new gene products.

Abstract

Preclinical research has indicated that transcranial near-infrared low-level light therapy (LLLT) administered post-traumatic brain injury (TBI) may provide a neuroprotective effect.



Objectives

This study aims to evaluate the feasibility and safety of acute LLLT administration following moderate TBI, as well as the therapy's neuroreactivity, assessed through quantitative magnetic resonance imaging (MRI) metrics and neurocognitive tests.



Design, Setting, and Participants

A randomized, single-center, double-blind, placebo-controlled, parallel-group trial was conducted between November 27, 2015, and July 11, 2019. The study included 68 men and women with acute, nonpenetrating, moderate TBI who were randomly assigned to receive either LLLT or a sham treatment. Analysis focused on the response-evaluable population.



Interventions

Transcranial LLLT was delivered using a custom-designed helmet within 72 hours post-trauma. MRI scans were conducted during the acute phase (within 72 hours), early subacute phase (2-3 weeks), and late subacute phase (approximately 3 months) of recovery. Clinical evaluations were performed concurrently and again at 6 months using the Rivermead Post-Concussion Questionnaire (RPQ), which consists of 16 items scored on a 5-point scale from 0 (no problem) to 4 (severe problem).



Main Outcomes and Measures

The primary outcome was the number of participants who successfully and safely completed LLLT without any adverse events within the first 7 days post-therapy. Secondary outcomes included the comparative impact of LLLT on MRI brain diffusion parameters and RPQ scores versus the sham treatment group.



Conclusions and Relevance

This randomized clinical trial demonstrated that LLLT is feasible for all patients and did not result in any adverse events. Light therapy significantly altered multiple diffusion tensor parameters during the late subacute phase. This study presents the first human evidence that LLLT interacts with neural substrates involved in the pathophysiological processes of moderate TBI, suggesting that diffusion imaging could serve as a biomarker for therapeutic response.
